Businesses and Non-Profits located within the City of Forest Lake who are able to demonstrate a financial impact due to the COVID-19 Pandemic are invited to apply. Full eligibility guidelines can be found here:

How to Apply

The application deadline for the CARES Act grant program has passed and applications are no long

​Grant Amounts
​
-
Up to Fifteen Thousand Dollars ($15,000) for employer-based business
-
Up to Seven Thousand Five Hundred Dollars ($7,500) for self-employed or non-employer home based business.

Up to Fifteen Thousand Dollars ($15,000) for Non-Profits
Priority funding will be given to for profit businesses (employer based and non-employer based) with remaining funds distributed amongst the non-profit applications on a first come first served basis.
FAQ's
Q: What documentation is required at time of application?
​
A: A completed Application Form with all questions answered. No additional documentation is required with the Application Form. If your application moves forward in the process, additional documentation will be requested.
At time of application, you will need to provide basic business information at the time of application including Federal Tax ID number, state employer number, and current legal business structure.
Q: My business had between 1-10 employees prior to the COVID crisis, but currently all employees have been laid off. Am I eligible to apply?

A: Yes. Eligibility is determined by the number of employees on March 1, 2020.

Q: I have received federal emergency funding through either the Paycheck Protection Program (PPP) or the Economic Injury Disaster Loan (EIDL), am I eligible to apply and receive City of Forest Lake CARES Act Grant funds?
​
A: Yes, but the City of Forest Lake CARES Act Grant funds must be used to cover different expenses over and above specific uses of federal funds.
Q: Are Non-Profits Eligible?
​
A: Yes non profits are eligible to receive funding. Non-Profits will be funded with the remaining funds after the For-Profit applications are reviewed.
Q: What documentation is required after the application, but prior to receiving a grant?
​
A: Documentation of business revenue from 2019 (or the business’ most recent fiscal year if different), e.g. tax return, financial statements, reports, or any such records that can verify business revenue.
Documentation of payroll and employee count, if any, as of March 1, 2020.
Grant agreement signed by authorized signers.
Q: I have applied for COVID-related emergency small business funding through Washington County, but do not yet know if I will receive funding. Should I apply to the City of Forest Lake CARES Act Grant Program?

A: Yes. Applicants are encouraged to apply for any and all sources of funding available. Businesses may only receive City of Forest Lake CARES Act Grant funds upon certification and verification that no other COVID-related emergency county or state funds have been received for same expenses.
